The Wellness Registered Nurse (RN) will work with an employer-based client performing virtual care and coordination. Approximately 10 hours virtual and 10 hours in-person working in a clinic setting. The key objective is to develop positive relationships by creating value through protocol-driven care, patient care virtually health coaching, care coordination and education. The Wellness RN performs duties and conducts interpersonal relationships in a manner designed to project a positive image of the client and the department. Candidate must be able to communicate effectively at all levels throughout an organization and ensure that all business is conducted in a timely, confidential, courteous & professional manner.
EDUCATION/EXPERIENCE: Graduation from an accredited school of nursing and a valid license to practice as a Registered Nurse is required in the state or states where clinics are located. Associate degree or higher required. A bachelor’s Degree or higher in Nursing is preferred.
LICENSURE, CERTIFICATION, and/or REGISTRATION . Current license as a Registered Nurse (RN) from the State where clinics are located and Basic Cardiac Life Support (BCLS) required. A valid driver’s license is required. In Wellness settings, the following is preferred: Certified Health Coach. In Occupational Health settings, the following is preferred: Certified Occupational Health Nurse (COHN), Certified Industrial Audiometric Technician, and NIOSH Approved Spirometry Training.
ESSENTIAL FUNCTIONS:
- Must be fluent in Spanish (both written and spoken)
- Provide remote nursing triage and support to participants selected by Client in states including North Carolina, South Carolina, and Georgia.
- Assist employees in obtaining appropriate care for needed services
- Provide coaching and education on lifestyle behaviors including healthy eating, weight loss, general wellness, stress management and cessation of tobacco, alcohol and illicit drug use
- Perform health-related research and provide education upon request (new diagnosis, diagnosis of family member);
- Assist in healthcare navigation
- Assist with connection to community based service
- Screen for social determinants of health, PHQ-2 and PHQ-9
- Facilitate connection to virtual services
- Referrals to existing benefits on the insurance plan
- Care coordination with primary care and specialists as needed
- Post-visit follow-up (internal and external appointments)
- Provides patient/family-centered care, acting as partner and adviser, assists and supports patients and families to optimally manage their health care, respecting their culture and values, individual needs, health goals, and treatment preferences
- Applies critical reasoning and astute clinical judgment in order to expedite appropriate care and treatment, especially given that the patient may be present with complex problems or potentially life-threatening conditions.
- Facilitates continuity of care using the nursing process, multidisciplinary collaboration, and coordination of appropriate health care services and community resources across the care continuum. Interacts with patients during face-to-face encounters or through various telecommunication strategies, often establishing long-term relationships.
- Focuses on patient safety and the quality of nursing care by applying appropriate nursing interventions, such as identifying and clarifying patient needs, performing procedures, conducting health education, promoting patient advocacy, coordinating nursing and other health services, assisting the patient to navigate the health care system, and evaluating patient outcomes.
- Provides leadership in the clinical setting and provides operational accountability for and coordination of nursing services, including the appropriate skill mix and delegation of roles and responsibilities for licensed and unlicensed nursing personnel.
- Designs, administers, and evaluates nursing services within the organization in accordance with relevant federal requirements, state laws and nurse practice acts, regulatory standards, and institutional policies and procedures.
- Participates in regular team meetings and continuous quality improvement activities
- Participates in strategic planning for onsite client events
- Performs and maintains competency for point of care testing as assigned, ensuring compliance with Atrium Health Point of Care policies and with applicable state and federal regulations
- Perform drug testing, Pulmonary function testing, Breath alcohol testing, audiograms, and fit testing
- Duties may require recordkeeping, actively participating in safety meetings throughout areas of support, educating patients on health-related topics, and establishing healthy working relationships with patients and site management
- Evaluates the employee’s medical condition and ability to perform specific job functions
- Appropriately document all medical examinations, tests, consultations, and evaluations
