A skilled Advanced Practice Provider is needed to join our AHWFB Pediatric Nephrology Department, where you will have the opportunity to make a meaningful difference in the lives of pediatric patients.

About the Role

This position offers a unique chance for an Advanced Practice Provider to work collaboratively with our nephrology team, providing comprehensive care to pediatric patients. Key responsibilities include:

• Delivering high-quality patient-centered care, including assessments, diagnostics, treatment planning, and management of conditions like kidney disease, hypertension, and electrolyte imbalances.
• Collaborating with nephrologists, PAs, and the nephrology team to develop and implement comprehensive care plans for both inpatient and outpatient pediatric patients.
• Supporting patients and families by offering education on kidney disease, treatment options, and lifestyle modifications.
• Assisting with the coordination of care, ensuring seamless communication among multidisciplinary providers.
