About the Role
This is an outstanding opportunity for an Advanced Practice Provider to work closely with our nephrology team, providing comprehensive care to pediatric patients. Key responsibilities include:
- Providing high-quality patient-centered care, including assessments, diagnostics, treatment planning, and management of conditions like kidney disease, hypertension, and electrolyte imbalances.
- Collaborating with nephrologists, PAs, and the nephrology team to develop and implement comprehensive care plans for both inpatient and outpatient pediatric patients.
- Supporting patients and families by offering education on kidney disease, treatment options, and lifestyle modifications.
- Assisting with the coordination of care, ensuring seamless communication among multidisciplinary providers.
