What are the responsibilities and job description for the Social Worker position at Wake Robin?
The Linden Health Center at Wake Robin, which is home to our Residential Care and Skilled and Long-Term Care neighborhoods, seeks a Social Worker to join our Health Services Team. The Social Worker reports to the Social Work Manager, and the Social Work Team is responsible for ensuring that the medically related emotional and social needs of the residents are met and maintained on an individual basis in accordance with policies and procedures and current state and federal regulations. The Social Worker must demonstrate effective communication, collaboration, organization, assessment, and problem-solving skills.

Qualifications
- Bachelor’s degree in Social Work or related field.
- Experience working with older adults.
- Experience with dementia care.
- At least 2 years of case management experience in a long-term care setting preferred.
