Job Overview:
Walkers Group Home is seeking a compassionate and organized individual to join our team as a Tailored Care Management (TCM) Care Manager. You will play a critical role in coordinating and managing comprehensive care for individuals with complex health needs. This position is ideal for someone with a strong background in care coordination and a commitment to improving the quality of life for individuals we serve, including those with mental illness, substance use disorders, and/or intellectual and developmental disabilities.
Key Responsibilities:
- Conduct comprehensive assessments to evaluate the health, social, and behavioral needs of clients.
- Develop and implement person-centered care plans in collaboration with clients, families, and interdisciplinary teams.
- Coordinate services with medical, behavioral health, and social services providers to ensure seamless care delivery.
- Monitor and adjust care plans based on client progress to achieve desired outcomes.
- Provide education and support to clients and families to help them navigate their care plans and available resources.
- Ensure culturally competent, person-centered service delivery that respects client preferences and values.
- Collaborate with community partners to address barriers to care, including housing, transportation, and employment.
- Document all services and interactions in compliance with agency policies and state and federal regulations.
- Participate in regular team meetings and care coordination conferences to discuss client progress.
- Advocate for clients in healthcare, judicial, and social service systems to ensure comprehensive and coordinated care.
Qualifications:
- Bachelor’s or Master’s degree in social work, nursing, counseling, or a related field.
- Minimum of 2 years of experience in care management, case management, or related fields.
- Strong understanding of tailored care management principles and ability to work with individuals with complex health needs.
- Excellent communication, organizational, and problem-solving skills.
- Experience working with diverse populations with a commitment to cultural competence.
- Valid driver’s license and reliable transportation.
- Ability to complete NCI-B and CPR training (required).
- Ability to lift up to 25 pounds and walk, stand, or stoop regularly.
