Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing personalized care plans, and coordinating resources to enhance the well-being of those we serve. The Case Manager will work collaboratively with various stakeholders to ensure that clients receive the necessary support in areas such as behavioral health, workforce development, and crisis management.
Responsibilities
- Conduct thorough assessments of clients to identify needs and develop individualized service plans.
- Provide supervision and mentorship to junior staff and interns within the case management team.
- Collaborate with healthcare providers, social services, and community organizations to coordinate care.
- Utilize motivational interviewing techniques to engage clients effectively in their recovery journey.
- Manage crisis situations by implementing appropriate interventions and support strategies.
- Assist clients in navigating Medicare and other benefits related to healthcare services.
- Provide guidance on addiction counseling resources and support for individuals facing substance use challenges.
- Facilitate access to housing assistance programs, including Section 8, for eligible clients.
- Maintain accurate documentation of client interactions, progress notes, and service delivery outcomes.
Qualifications
- Must have a high school diploma.
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field is preferred.
- Experience in behavioral health or addiction counseling is highly desirable.
- Strong understanding of workforce development principles and practices.
- Proven ability to manage crises effectively while maintaining client dignity and respect.
- Excellent communication skills with the ability to build rapport with diverse populations.
- Familiarity with childcare resources and support systems is a plus.
- Ability to work independently as well as part of a multidisciplinary team.

Pay: $23.00 - $25.00 per hour
