Key Responsibilities:
Personalized Resident Assessments:
- Take the time to deeply understand each resident’s social, emotional, and psychological needs through comprehensive assessments.
- Collaborate with interdisciplinary teams to continually monitor and update care plans based on residents’ evolving needs.
Care Plan Architect:
- Be the voice of the residents in the care planning process, ensuring their emotional and mental health needs are seamlessly integrated into their physical care.
- Partner with medical professionals to create truly individualized care plans that prioritize both well-being and comfort.
Heartfelt Support & Counseling:
- Provide compassionate counseling to residents and their families, particularly during difficult transitions like admission, end-of-life, or discharge.
- Offer a safe space for grief support, conflict resolution, and mental health counseling, addressing the unique challenges of each individual.
Case Management Extraordinaire:
- Navigate and manage complex case situations by connecting residents to essential community resources, financial assistance, and social services.
Family Partner & Educator:
- Be a trusted partner for families, offering guidance, education, and emotional support through every step of the process.
- Facilitate family meetings to ensure a collective approach to care, making sure all voices are heard and respected.
Resident Advocate & Protector:
- Stand as a strong advocate for residents' rights, ensuring their dignity and needs are prioritized.
- Act quickly and responsibly in identifying and addressing any concerns related to abuse, neglect, or rights violations, reporting as required.
Meticulous Documentation:
- Keep detailed, timely, and precise records of interactions, assessments, and care plans, ensuring compliance with all legal, ethical, and facility standards.
- Your documentation supports the best possible care decisions and contributes to the smooth functioning of the facility.
Seamless Discharge Planning:
- Lead the charge in discharge planning, ensuring residents have the resources and support they need when transitioning out of the facility.
- Ensure residents are empowered with the information and community connections they need to thrive after their stay.
Collaborative Team Player:
- Work closely with our interdisciplinary healthcare team to ensure that care plans evolve in real-time based on the residents' changing needs.
- Attend meetings and contribute meaningful insights to make each resident’s experience exceptional.
What You Bring:
- Education: A bachelors degree in Social Work from an accredited program.
- Licensing: You hold a Licensed Social Worker (LCSW) or equivalent from PA
- Experience: Preferably 1-2 years of experience in social work within a healthcare, long-term care, or SNF environment.
- Skills:
- Empathy and emotional intelligence that helps you connect with others at a deep level.
- Ability to juggle multiple priorities while keeping the resident's well-being at the forefront.
- Strong problem-solving and communication skills that help you navigate complex situations with grace and professionalism.
