What are the responsibilities and job description for the Case Manager position at Walnut Street Center?
Walnut Street Center, Inc is Nonprofit, Human Services agency providing support to Adults with Intellectual Disabilities. We are currently seeking a Case Manager to join our growing team.
The Case Manager provides supports for adults with developmental disabilities who are living independently or with their families. Such support services are determined through the individual’s ISP and in ongoing clinical and functional assessments. The goal is to promote individual growth in the areas of choice and decision-making, community membership, household management skills, relationships, and personal accomplishments. This position will ensure that the individuals achieve/maintain independence and control of their lives. The Case Manager balances his/her schedule of visiting individuals in their homes and working in the office taking phone calls from individuals, coordinating services, and participating in team meetings.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Case Management Skills
- Balances a regular caseload dependent upon each individual’s hours of support. Maintains a regular schedule of interactions with individuals on his/her caseload.
- Provide skill training to both individuals and outside staff, medical and benefits coordination, supportive counseling, and crisis intervention as directed by the individual. Identify unmet needs and participate in the development of plans to address those needs. Work cooperatively/ collaboratively with other community service providers and medical professionals to ensure coordination of supports and services.
- Promote activities and opportunities leading to personal growth and accomplishment. Facilitate and coordinate, with the individual, access to needed supports including generic community services.
- Develop a therapeutic relationship with individuals. Provides supervision to ensure safety and well- being of individuals. Provides supports that promote independence through skill building programs. Encourages participation and is helpful, supportive and respectful of differences with regards to unique needs, age, gender and culture of the individual being supported.
- Assists with the development, implementation, evaluation and documentation of Individuals Treatment Plans (ISP, ect). Provides case management as assigned; assessing individual’s skill set and areas of need, participating in the development of treatment Plans and evaluating the effectiveness of the goals, objectives, plans and procedures. Implements formal skills building programs, as identified in a plan.
- Supports and monitors individual funds as appropriate. Provides guidance/support to ensure financial needs are addressed.
2. Provides therapeutic support
- Completes behavior modifying medication assessments on an as-needed basis.
- Effective Interaction. Provide support and assistance to individuals necessary for them to develop and maintain relationships with others.
- Human Rights. Promote individual choice and self-expression based upon quality service principles. Ensure that individuals are treated with respect and dignity at all times, and maintain confidentiality regarding their records and other information.
3. Provides Safe Environment
- Providing oversight of individual’s home environment to ensure compliance with basic safety practices. Participates in the planning process to ensure adequate support in the areas of health and safety.
- Transportation. Provide transportation as appropriate in response to individuals’ schedules and desired activities and/or appointments. Uses agency-provided vehicles and public transportation.
4. Provides for community integration and promotes a good public image.
- Community relations. Promotes a favorable image of individuals and the organization by fostering good community relations by demonstrating appropriate professional behavior.
- Community access. Provides supports to ensure access to community, integrating opportunities for new experiences as appropriate.
- Notification. Notifies appropriate supervisor in the event of a community incident or any issue that could affect the individual, program or organization.
5. Professional responsibility
- Maintains professional relationships withco-workers, management, guardians and members of the community in a professional manner. Is responsive to constructive feedback and adjusts accordingly.
- Exercises good judgment. Initiates course of action that follows WSC Policies, Procedure and Protocols. Seeks additional information from management if course of action is not established. Resolves conflict in a productive manner and/or consults management for assistance
- Effectively responds to emergency situations; makes sound independent decisions, seeks information and clarification , resolves conflict in a productive manner
- Additional responsibilities. Performs other duties, responsibilities and projects as assigned by supervisor or management
6. Documentation/Communication.
· Documents and notifies management of any issues brought forth that effect the therapeutic care or environment
· Participates in meetings and case reviews.
7. Quality improvement. Participates in continuous improvement to increase productivity, reduce inefficiencies and enhance the quality of services provided.
8. Additional responsibilities. Performs other duties, responsibilities and projects as assigned by supervisor or management.
Qualifications:
- Associate's degree preferred
- Valid drivers license required
- Covid19 vaccination card
- Ability to pass background checks
- Ability to communicate effectively in English
- Ability to independently complete computer based training
