Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN, you will work under the supervision of registered nurses and physicians, assisting in the delivery of healthcare services to patients across various settings.
Duties
- Provide direct patient care, including administering medications and treatments as prescribed.
- Assist in discharge planning and coordinate with healthcare teams to ensure continuity of care.
- Monitor patients’ vital signs and report any changes to the supervising nurse or physician.
- Maintain accurate patient records using electronic health record systems such as eClinicalWorks.
- Support patients with disabilities by providing personalized care tailored to their needs.
- Participate in coding and documentation for billing purposes.
- Collaborate with multidisciplinary teams in pediatrics, long-term care, hospital medicine, and behavioral health settings.
- Administer botulinum toxin treatments as required, ensuring adherence to safety protocols.
- Assist with dialysis procedures when necessary, ensuring patient comfort and safety.
Requirements
- Valid LPN license in the state of practice.
- Strong clinical skills with experience in discharge planning, pediatrics, long-term care, or behavioral health preferred.
- Familiarity with electronic health record systems, particularly eClinicalWorks.
- Ability to work effectively with individuals with disabilities and provide compassionate care.
- Knowledge of coding practices related to nursing documentation is a plus.
- Experience in administering botulinum toxin treatments is desirable but not mandatory.
- Excellent communication skills and the ability to work collaboratively within a healthcare team.
- Commitment to maintaining patient confidentiality and adhering to ethical standards in nursing practice.
