Walworth County, WI is seeking a qualified candidate for the position of Care Specialist - Access and Support Professional. This role plays a crucial part in providing information and assistance to the general public, particularly older adults, adults with disabilities, and youth. The successful candidate will work within the Aging & Disability Resource Center, collaborating with other programs and individuals to ensure coordination of care.
Key Responsibilities:
- Facilitate consumer access to services by working closely with contracted providers and monitoring purchased services.
- Evaluate and assess individuals and families with psychosocial and/or physical difficulties, developing and carrying out service plans as needed.
- Administer functional and financial screens to determine program eligibility and provide short-term case management services for assigned caseloads.
- Complete annual protective placement reviews, access and abuse neglect reports, and advocate for consumers and their support systems.
Requirements:
- Bachelor's degree in social work or a related field, with at least one year of relevant prior experience required.
- Master's degree preferred, along with experience working with aging and disabled populations.
- Knowledge of long-term care programs, such as Family Care, IRIS, AFCSP, SSIE, and MA-covered services preferred.
- Certification as an I&R Specialist in Aging/Disability (CIRS-A/D) required within one year of hire, while Wisconsin DHS Adult Long Term Care Functional Screener certification is preferred at time of hire or must be obtained within two months of hire.
Benefits:
- Full-time employment status, with eligible employees receiving overtime or compensatory time in accordance with provisions of the Fair Labor Standards Act.
- Accrued based on straight-time hours paid each pay period, with 10 days per year after 1 year of Walworth County Service, 15 days per year after 5 years of Walworth County Service, and 20 days per year after 15 years of Walworth County Service.
- 64 hours of personal time off as a lump sum at the beginning of each year, with the option to move remaining hours to the extended sick leave bank.
