Hourly Wage range:
LNA per diem rates $19.00 to 22.89 - Depends on years of experience
Per Diem requirement is working a minimum of 4 shifts a month
2 of the 4 shifts must be a weekend Saturday and/or Sunday.
Additional Shift Differential:
Applied on top of the hourly rate -
Weekday $2 for 2nd and 3rd shift
Weekend $3.00 for 2nd shift
Weekend $3.50 for 3rd shift

Warde Senior Living in Windham, NH, is currently a 39-bed Assisted Living community and a 32-bed skilled nursing facility offering short and long-term care in a warm, supportive atmosphere where residents are cared for according to their individual needs. In addition to standard LNA duties, job responsibilities include but are not limited to:
• Participate in and receive report from the licensed nurse at the beginning and end of assigned shift
• Maintain awareness of care plans daily to ensure the provision of appropriate care
• Observe residents frequently during assigned shift to ensure all personal care needs are being met
• Maintain confidentiality of all resident-related care information to ensure HIPAA compliance
• Treat residents with kindness and dignity while respecting resident rights and privacy at all times
• Encourage residents to develop and maintain independence and highest level of functioning in all activities of daily living
Requirements
· Must be age 17.5 or older
· Must have High school graduate or GED
· Graduate of an approved Nursing Assistant Training Program and currently licensed as a Nursing Assistant in good standing with the State of New Hampshire Board of Nursing
