We are seeking a compassionate and detail-oriented Registered Nurse (RN) to join our Home Health team. The RN will provide skilled nursing care to patients in their homes, ensuring high-quality, individualized care that promotes comfort, dignity, and healing. This position involves close collaboration with patients, families, physicians, and other interdisciplinary team members to develop and implement care plans tailored to each patient’s needs.

Key Responsibilities:

• Conduct in-home nursing assessments and develop personalized care plans.
• Provide direct patient care, including wound care, IV therapy, medication administration, catheter care, and chronic disease management.
• Monitor patient status and update care plans as needed in collaboration with physicians and other healthcare providers.
• Educate patients and families on disease processes, treatment plans, medications, and self-care techniques.
• Document all care provided in accordance with agency policies, regulatory guidelines, and electronic medical record systems.
• Coordinate with other members of the care team including physical therapists, occupational therapists, social workers, and home health aides.
• Adhere to infection control, safety, and documentation standards.
• Participate in case conferences and continuous quality improvement initiatives.
• Maintain regular communication with the home health agency, patients, and caregivers.

Qualifications:

• Current, unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience; home health experience strongly preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Strong clinical skills and sound nursing judgment.
• Proficiency with electronic documentation and technology.
• Excellent communication, organizational, and time management skills.
• Ability to work independently in a home setting with minimal supervision.

Work Environment:

• Patient homes in various community settings.
• Driving required for patient visits.

Schedule:

• Full-time / Part-time / PRN positions available.
• Flexible scheduling depending on patient needs and agency requirements.

The purpose of this position is to participate in nursing care by planning, coordinating, providing and evaluating care for clients in their homes, including administering medications, doing treatments, assessing clients, communicating with physicians and families, directing direct care staff for the client’s well-being.

• Work Environment & Physical Requirements:

  • Requires frequent travel to patient homes and clinical sites.
  • Must be able to lift, push, pull, and transport medical equipment and supplies as needed up to 50 lbs.
  • Ability to work flexible hours, including occasional evenings and weekends, as required for patient care needs.
  • Able to work in a team setting and independently as well.
  • Must be comfortable working with diverse patient populations, including those with chronic illnesses or disabilities.
  • Graduate from an accredited professional school of nursing.
  • A valid and current RN License in good standing from the Virginia State Board of Nursing.

Compensation:
