Job Summary : The Practice Facilitator is responsible for working with CMS directives, health plan initiatives, systems and practices to facilitate practice redesign efforts, utilizing innovative thinking about how to best support achievement towards improved process and clinical outcomes, decreased costs, and increase in efficiency that results in quality measure attainment and provider and health care team satisfaction.Minimum Education : High School Diploma or GED. Bachelor's degree, preferred. Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom, preferred. Licensure, Registration and / or Certification : A valid driver's license, Motor Vehicle Report, and proof of vehicle liability insurance in the amount required by SFHS guidelines. Valid multi-state or State of Oklahoma Registered Nurse License, preferred. Work Experience : 5 - 7 years related experience. Knowledge, Skills and Abilities : Proficient knowledge of Microsoft Outlook, Access, Word, and Excel. Working knowledge of Epic Practice Management software and medical record documentation, documentation software, and concepts of quality reporting. Working knowledge of Health System and Medical Office policies and procedures. Effective interpersonal, written and oral communication skills. Strong presentation and delivery skills. Ability to prioritize work in an effective and efficient manner. Ability to problem solve and exhibit initiative in analysis and teamwork activities. Essential Functions and Responsibilities : The Practice Facilitator serves as the primary liaison, teacher, coach, and facilitator for practices participating in CMS quality initiatives as well as health plan practice transformation activities. Establishes and maintains a supportive, collegial role with assigned practices, and their individual quality improvement team members. Coaching practices using a standardized approach based on current quality initiative programs and approved workflows. Provide organizational support through disseminating experience- related learning and best practices, by way of team updates, written reports / articles, developing reference guides, toolkits and / or presentations. Link practices to available external resources, such as consultants and group learning opportunities, to meet specific needs which extend beyond those that are met by the Practice Facilitator's role. Train, develop, and educate the leaders, providers, and staff in practices; and identify interventions with support from the manager to facilitate success in practices. Develop innovative tools and resources, such as toolkits, guides, and presentations to meet current or future practice needs, and contribute to the learning community. Identify innovative curriculum, products, and services, and bring them to the attention of management. Demonstrates the ability to effectively lead and facilitate improvement teams toward measurable outcomes with increasing practice independence over time. Assists practices in maximizing use of available health information technology resources in order to accomplish improved patient outcomes and increased efficiency. Routinely assess, document, and report practice progress throughout the transformation process and proactively bring practice issues to the manager. Maintain databases directly associated with role. Decision Making : Independent judgment in making decisions involving non-routine problems under general supervision. Working Relationships : Works with internal and / or external customers via telephone or face to face interaction. Works with other healthcare professionals and staff. Works frequently with individuals at Director level or above. Special Job Dimensions : Requires occasional travel.
