Job description
Warrenton Woods is a residential addiction treatment center for adult males located on 50 acres in Warrenton, Virginia. The peaceful, wooded setting provides our clients with the perfect environment for treatment and recovery. We are currently seeking an experienced and resourceful case manager to join our compassionate and dedicated team. This is a great learning opportunity for a CSAC Supervisee needing clinical hours and professional development. The position includes tasks in all 12 core functions required for the CSAC supervised experience. Board approved clinical supervision is provided.
Job Overview
The Case Manager is a vital member of the treatment team. The position's primary role is to identify available resources, seek out new resources, and coordinate all activities related to the clients’ discharge/continuing care plans. The Case Manager also guides clients through the intake process and assists clients with identified immediate issues.
Responsibilities
- Based on clinical recommendations and approval, develops appropriate discharge/aftercare plan specific to the client’s needs, coordinating with referral sources, other treatment programs, medical providers, psychiatrists, and individual therapists to ensure ongoing recovery for the client
- Guides client through the intake process and completes all intake documentation including informed consents, releases of information, demographic information, and any needed clinical assessments
- In conjunction with the client and primary counselor, completes case management plan within the specified timeframe
- Coordinates interdepartmentally to facilitate approved external medical, social services, and legal appointments
- Initiates and maintains ongoing contact with any legal/court contacts on behalf of client to prevent any further legal stress or charges while in treatment
- Follows all ethical and regulatory guidelines
- Educates clients and their families on available services and resources
- Develops and conducts educational groups related to living skills, recovery skills, and aftercare
- Documents all client-related activities and observations in the electronic clinical record
- Performs other duties as requested.
Requirements
- Bachelor's degree, preferably in a human services field
- Preference given to CSAC Supervisees or those who can quickly become eligible for supervisee status
- Proven experience in case management, preferably with an addiction treatment/recovery facility
- Highly resourceful, creative, and able to think outside the box
- Ability to work collaboratively with interdisciplinary teams
- Ability to work independently to overcome challenges
- Knowledge of related physical behavioral health terminology
- Excellent English verbal and written communication skills
- Must be able to speak clearly and concisely over the telephone
- Good time management skills
- Comfortable working with technology
- Ability to quickly adapt to and support challenges and changes at the facility
