We are a trauma-informed, gender-specific residential treatment facility dedicated to supporting women in their recovery journey from substance use disorders. Our program focuses on individualized care, empowerment, and long-term recovery support through a comprehensive continuum of services.
Position Summary:
We are seeking a compassionate, organized, and motivated Case Manager / Alumni Coordinator to join our team. This dual-role position provides essential case management services to current clients and maintains connection and engagement with program alumni to support ongoing recovery and community building.
Key Responsibilities:
Case Management Duties:
• Develop and implement individualized service plans for clients
• Coordinate services including mental health, medical, legal, housing, employment, and family support
• Facilitate case conferences and collaborate with interdisciplinary team
• Maintain accurate documentation and timely progress notes
Alumni Coordination Duties:
• Establish and maintain contact with program alumni
• Plan and coordinate alumni events, support groups, and workshops
• Collect and report on alumni outcomes and feedback
• Foster peer mentoring and recovery support initiatives
Qualifications:
• Bachelor’s degree in Social Work, Psychology, or related field (preferred)
• Minimum of 2 years of experience in substance abuse or mental health services
• Knowledge of trauma-informed care, recovery principles, and community resources
• Excellent communication and organizational skills
• Ability to build rapport with diverse populations
• Valid driver’s license and ability to pass background check
