Description
SUMMARY
This position provides diabetes education and diabetes management for patients. Participate as a member of the health care team and as directed by referring physicians to meet patient needs and provide continuity of care.
To perform this job successfully, an individual must be able to perform each responsibility satisfactorily. The requirements listed below are representative of the knowledge, skill, and / or ability to be successful.
DUTIES / RESPONSIBILITIES (include but are not limited to the following)
- Provide direct care, education, and counseling for inpatients and outpatients as directed by patients physicians.
- Participate as a member of the multi-disciplinary team to develop, implement, and evaluate individual care plans.
- Document care and activities in the patients medical record in a clear, complete, concise, and timely manner.
- Serve as a patient advocate to resolve concerns or complaints.
- Responds to appropriate medical emergencies.
- Assist in the development, and implementation of diabetes-related programs and materials for patients, staff, community, and other professionals.
