Position Overview
We are seeking a skilled and compassionate Youth Support Services Provider to join our team at Washington County Mental Health Services. As a youth support services provider, you will play a critical role in supporting individuals 0 - 22 years old with intellectual disabilities and their families.
About the Role
The successful candidate will be responsible for coordinating and implementing personalized treatment plans, developing services that are family-centered, and ensuring compliance with disability support guidelines. This role involves acting as a liaison among team members, facilitating communication, providing technical assistance, attending school IEP meetings, and coordinating access to medical, vocational, and recreational resources.
Responsibilities
- Develop and implement individualized treatment plans in collaboration with the multidisciplinary team
- Provide education and support to individuals and their families regarding their condition and available resources
- Coordinate access to community-based services, including medical, vocational, and recreational activities
- Maintain accurate electronic medical records and communicate effectively with team members
- Participate in on-call coverage and respond to emergency situations as needed
Requirements
- Bachelor's degree in a related field (e.g., social work, psychology, special education)
- Three or more years of experience in developmental services/mental health services with a diverse population preferred
- QDDP status preferred
- Valid driver's license, excellent driving record, and access to a safe, reliable, insured vehicle
- Ability to triage crisis interventions, be flexible and mobile in response
- Excellent written and oral communication skills
