This position will primarily serve youth and young adults with complex behavioral health needs, with the primary goal of improving health outcomes through collaboration with the individual, system providers and partners, and other natural supports. About The Opportunity
The Intensive Care Coordinator (ICC) will work to ensure Oregon Health Plan (OHP) members residing in Washington County have access to care coordination regardless of eligibility for specific services. In this position, you will work with youth and transitional-aged young adult clients to provide needs assessment, care planning, connection to service delivery systems, and coordination with behavioral health, physical health and dental providers. Using a person-centered approach, you will schedule and facilitate interdisciplinary team meetings and coordinate with partners including legal systems, Department of Human Services (DHS), Aging and People with Disabilities (APD), and Developmental Disability services with the goal of improving communication and health outcomes. You will also maintain written documentation of all contact with the clients, their family, and system and service providers.
This position may be eligible for a hybrid work schedule.
