Position Overview: We are seeking an organized and compassionate Care Plan Coordinator (CPC) to join our dedicated team. The CPC plays a vital role in ensuring the timely and accurate completion of resident assessments and the development of personalized care plans. This role requires strong communication and documentation skills, as well as the ability to work collaboratively with an interdisciplinary team, including residents, families, and staff.
Key Responsibilities:
- Oversee the completion of comprehensive assessments, MDS assessments, and care plan development for each resident.
- Collaborate with interdisciplinary team members (nurses, doctors, social workers, etc.), residents, and their families to gather relevant information for developing individual care plans.
- Develop, implement, and regularly review the resident’s Plan of Care, ensuring it meets the needs and preferences of the resident.
- Communicate care plan details to all relevant staff, ensuring thorough understanding and implementation of care directives.
- Monitor and document resident progress, ensuring appropriate follow-up and adjustments to care plans as necessary.
- Observe daily resident care to ensure adherence to care plans and provide support where needed.
- Serve as a liaison for family members, ensuring they are kept informed and involved in the care process.
- Ensure compliance with all regulatory and quality standards.
Qualifications:
- Must be of legal working age.
- Strong communication skills, both written and verbal, with the ability to document and articulate care plans clearly and effectively.
- Ability to work well within an interdisciplinary team and communicate with residents, families, and staff in a professional, compassionate manner.
- Experience with or knowledge of MDS assessments and care plan development is preferred.
- Must possess a valid nursing license in good standing in the state where the facility is located.
- Ability to maintain confidentiality and adhere to privacy standards.
- Ability to deal tactfully and professionally with personnel, residents, families, visitors, and regulatory agencies.
- Willingness to work overtime when needed.
