What are the responsibilities and job description for the Case Manager position at Washington Square Services Corporation?
Responsibilities
· Provides case management services to residents of McKinney Cooperative Shelter programs transitioning from homelessness to permanent housing.
· Conduct thorough assessments of clients' needs, goals, and current circumstances to develop individualized care plans
· Works with residents to develop and monitor individualized housing goals and plans.
· Coordinates and monitors services by including comprehensive tracking of client activities in relation to the care plan.
· Assists residents in identifying and addressing personal issues
· Assists residents with ADL and vocational skill enhancement efforts
· Coordinates resident advancement through McKinney Shelter programs.
· Satisfies all reporting and record keeping requirements.
· Maintains confidentiality standards.
· Liaisons with social service, law enforcement, and government agencies.
· Coordinates and monitors services by including comprehensive tracking of client activities in relation to the care plan.
· Adheres to professional standards as outlined by protocols, rules, and regulations
· Develops effective working relations and high levels of collaboration by cooperating with local partnerships and with both state and local agencies.
· Performs other duties as assigned.
Qualifications
2-5 years of prior related experience and knowledge of homelessness, mental illness, and substance abuse. BSW or appropriate combination of education/training in psychology, social work, mental health or related field preferred.
Status: Full-time, hourly position with benefits.
Some evening and weekend hours expected.
