Position Summary
This individual will serve a valuable role within the Division of Infectious Disease by assisting patients with their unmet social needs in order to prioritize their HIV care. Our mission is impact lives through innovation and excellence in HIV care, prevention and support. The community health worker will work directly with individuals living with HIV and their families, engaging in community outreach, educating clients about their disease, providing navigation around the hospital and clinic, and lending support to patients as they engage in care.
Job Description
Primary Duties & Responsibilities :
- Provides support to clients by assisting with navigation of the medial system, education, and emotional support.
- Attends client medical appointments to support adherence
- Meets clients in the community when appropriate
- Performs outreach in the community to inform community members, in culturally relevant language, about research opportunities available at the University.
- Refers community members to available resources, from medical and social services to opportunities offered.
- Helps with arranging language translation or interpretation services as needed.
- Assists in arranging transportation to medical appointments.
- Helps with referrals to resources to reduce barriers.
- Assists in building partnerships with community agencies and resources
- Participates as a member of the multi-disciplinary team.
- Maintains proper documentation and record keeping.
