Wound Care - RN *Per Diem
*IMPORTANT NOTE: In lieu of benefits due to "per diem" status, 15% will be added to the hourly rate. Per diem employees are offered work on an "as-needed" basis.
POSITION SUMMARY:
Manages individualized, goal-directed nursing care through use of the nursing process and the principles of primary nursing in accordance with departmental and hospital policies and procedures; provides age appropriate care from adolescents through the geriatric patient based on the population served; assesses, plans, implements and evaluates care in collaboration with the multidisciplinary care team; demonstrates the knowledge and skills necessary to identify and meet the individual needs of all patients served.
POSITION REQUIREMENTS:
Minimum Education:
- Minimum of an Associate’s Degree in Nursing from an accredited school of Nursing.
Minimum Education Preferred:
- Bachelor's Degree preferred.
License Required:
- Current Nevada RN license.
Certification Required:
- AHA BLS.
- Wound care certification required within 6 months.
Minimum Work Experience:
- Two (2) years minimum nursing experience is preferred.
- Previous wound care experience and certification preferred.
- Professional customer service skills.
- Ability to read and communicate in English, bilingual abilities preferred.
POSITION ESSENTIAL FUNCTIONS:
Patient Care
- Assesses and diagnoses patient and family needs in order to provide quality care to assigned patients.
- Performs admission assessment upon admission or in accordance with specific unit standards.
- Identifies and documents patient's needs and treatment plan upon admission.
- Identifies patients requiring wound care who are high risk, and will require long term treatment or who have other critical needs.
- Identifies and documents patient/family/significant other teaching needs relative to wound management upon admission and as needed.
Treatment Plan
- Develops, discusses, and communicates a realistic treatment plan for each patient, in collaboration with each patient/family/significant other in order to address all identified needs.
- Develops comprehensive wound assessment and plan of care including goals, implementation of treatment, evaluation and outcomes of the patient, including those with special needs.
- Recommends revisions to the plan of care based upon the patient's response to treatment
- Evaluates overall plan for effectiveness.
- Collaborates with the physician and other healthcare professionals and departments to ensure the treatment is implemented and carried out successfully.
- Evaluates the identified problems, care provided and the patient's response in order to meet patient and family needs to ensure overall quality of care delivered.
- Reassessment of effectiveness of treatment plan and interventions performed with every patient visit.
- Assesses the patient/significant other's response to teaching plan in order to evaluate the effectiveness of the plan.
Judgment
- Demonstrates the skills and judgment necessary to implement medical treatment plan, nursing interventions, and procedures as necessary for the care of the patient.
- Demonstrates knowledge of commonly used medications: action, dose, and side effects.
- Demonstrates proper procedures in all methods of medication administration.
- Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Demonstrates problem solving skills through knowledge, research, and practice.
- Demonstrates knowledge of hospital policies and procedures for administering, transcribing, and recording medications.
- Observes 10 rights of medication administration: right med, right dose, right time, right route, right patient, right education, right documentation, right to refuse, right assessment, right evaluation.
Documentation
- Maintains up-to-date and accurate documentation of nursing care provided to ensure the integration of information for use by the healthcare team to ensure quality care.
- Nursing assessment is complete and accurate per unit standard.
- Nursing documentation is reflected in the wound care progress notes with an entry made per unit standard and when reassessment of the patient requires. Including photo’s of wound at least weekly.
- Nursing documentation reflects current plan for the patient and updates.
- Collects information to evaluate program quality and outcomes as defined in the department quality improvement plan.
Scope of Service
- Performs all responsibilities/duties required by unit as defined in the scope of service, to ensure that the unique nature of the client is addressed. This includes, but is not limited to the age of the client served.
- Assesses, evaluates, and carries out nursing interventions and teaching in wound patients of all ages.
- Demonstrates skill and knowledge of anatomy and physiology and disorders of the gastrointestinal, genitourinary and dermal systems.
- Demonstrates knowledge and skill in use of special equipment, instruments and devices utilized for wound care.
- Utilizes principles of current wound management theory when providing care to patients with acute and chronic wounds.
- Demonstrates knowledge and understanding of wound assessment, reassessment and management requirements for problem wounds.
- Demonstrates thorough knowledge and skill in staging system of wounds.
- Demonstrates skin care programs to prevent skin breakdown of patients.
- Demonstrates knowledge of surgical procedures and postoperative course of patients.
- Demonstrates knowledge and skill to develop and implement policies and procedures to manage patients with draining wounds, fistulae and/or tubes; wound prevention, wound care, ostomy care and incontinence.

Wound care is open Mon-Fri. Per Diem positions have no guaranteed hours or set schedule. The position will fill in for individuals who call in sick, take vacation or take a leave of absence.
