JOB SUMMARY:
To assist in the provision of physical therapy to patients in their home, under the supervision of a licensed physical therapist, according to established Agency policies and procedures.
Promotes high quality standards of care and positive outcomes
Reports to Therapy Supervisor
QUALIFICATIONS:
Minimum Education and Experience
Graduate of Physical Therapy Assistant Associate Degree program accredited by an agency recognized by the Secretary of the United States Department of Education or the Council on Postsecondary Accreditation.
Experience: One year of acute care experience. Additional experience in home health care preferred.
Other Qualifications
A valid driver's license and personal vehicle
Certification by the State of Connecticut.
Physical, mental and sensory abilities sufficient to meet performance standards with or without assistive devices
