QUALIFICATIONS:
Minimum Education and Experience
- Graduate of a school approved by the Council on Medical Education of the American Medical Association. in collaboration with the American Occupational Therapy Association, plus a minimum of three (3) years clinical experience in occupational therapy.
Other Qualifications
- A valid driver's license and personal vehicle
- Current license in the State of Connecticut to practice Occupational Therapy
- Physical, mental and sensory abilities sufficient to meet performance standards with or without assistive devices
PERFORMANCE CRITERIA:
VNA Health at Home, being the principal provider of services, is responsible to coordinate all Occupational Therapy services provided under contractual arrangement or directly by a staff therapist.
The Occupational Therapist will possess the following skills and attributes and be responsible for the following duties:
- Provides home occupational therapy services to VNA Health at Home patients in accordance with current requirements of agreements/contract.
- Admits patients to service and administers individual therapy programs following the prescription of the referring physician in accordance with VNA Health at Home policies and procedures
- When patients are also receiving nursing services, the therapist works with the primary care nurse in the development of the care plan and coordination of services
- Performs comprehensive evaluation of patient's level of function and participates in development of the total patient care plan
- Identifies patient and family needs for other home health services and initiates referrals for same when needed
- Instructs and supervises home health aides and family members in accordance with plan of care and records in progress notes to VNA Health at Home.
- Communicates the plan of care, progress and discharge plans to the RN Case Manager, Rehabilitation Coordinator, and physician in accordance with agency policies
- Submits appropriate data for computerized visits within the time frames specified by agency policies and procedures
- Case conferences with primary care nurse according to agency policy
- Obtains physician orders for care provided. Obtains insurance authorization whenever necessary
- Participates in Peer Review in accordance with state regulations.
- Attends educational programs, in-services and seminars to further education and professional development.
- Strives to prioritize and manage time effectively in order to maximize timely completion of tasks and goals.
- Provides supervision of home health aides as warranted by the plan of care.
- Participates in the agency's Quality Improvement Program. Identifies areas for improvement within agency processes as well as personal/professional learning needs. Regularly attends inservices or outside seminars to enhance practice and knowledge.
