Job Description
Job Purpose :
Under the supervision of the Behavioral Health Administrative Supervisor (BHAS), the CBHC provides both clerical and clinical coordination with multiple agencies and providers in the community to connect patients with appropriate Behavioral Health care and resources programs. The CBHC will coordinate with the BHAS on the formulation and implementation of public health initiatives, have awareness of community resources, organize and execute public relations outreach, develop and deliver presentations, manage website design and content, and be responsible for patient care coordination.
Duties / Responsibilities :
- This position will create and maintain relationships with community stakeholders, providers, and patients in Coos and Curry; ensuring professional, accurate, and prompt responses to inquiries and requests.
- Responsible for proactively contacting and convening opportunities for collaboration between the different agencies and participants in the Hub support initiatives identified by Coordinated Care Organizations and Oregon Health Authority.
- Assist patients in referral process, maintain up-to-date profile information on multiple providers, request documents from community agencies to coordinate care, and manage feedback from patients and providers.
- Case management for participants as needed, including conducting screening phone calls to determine participant interest and eligibility for referrals for Behavioral Health programs.
- Answers patient inquiries to streamline the process for patients and refers appropriate clinical questions to clinical personnel, other departments, and / or community resources as necessary.
- Coordinate sponsored events and activities and represent behavioral health programming in the community.
- Collaborate with behavioral health leaders and marketing team on outreach strategies, including public relations campaigns, evaluate effectiveness, and make adjustments based on community feedback.
- Implement communication strategy, including design and maintenance of Coos and Curry Behavioral Health Hub website (CCBH Hub), promote up-to-date marketing materials, report data analytics, and oversee online content management.
- Perform presentations to stakeholders, community groups, and agencies, conduct outreach, recruitment, and serve as the provider / agency liaison for the CCBH Hub.
- Partner with CBHO on budget, fund allocation, and submission of grant applications.
- Provide monthly reports and analytics on CCBH hub utilization.
- Develop and implement the DHS TANF Program including invoices, reports, and provide direct supervision and support the Mental Health Step Case Manager, maintain consistent communication with DHS administrators, family coaches, and step participants.
- Adheres to all clinic protocols regarding the management and compassionate care of patients undergoing mental health crisis.
- Demonstrate awareness and sensitivity in the delivery of activities and events as it relates to the cultural and socioeconomic characteristics of the populations served.
- Ability to adapt and respond with communication strategies to stabilize, slow, or reduce the intensity of a potentially high stress situation.
