What are the responsibilities and job description for the Family Practice/Primary Care Nurse Practitioner position at Waterfall Community Health Center?
Under the supervision of the Chief Medical Officer, the Nurse Practitioner, functions in a team and mission-based setting to deliver primary care. This includes health promotion / maintenance, counseling, education, assessment, acute / chronic care, and consultation / collaboration with supervising physicians, other health care professionals, and community resources. Exercises progressive levels of autonomy in day-to-day activities based on satisfactory job performance. To be successful, you should be kind, compassionate, and caring.

Job Responsibilities

Obtains and records client's health history; performs comprehensive physical assessment (including biological, social, and psychological needs) as part of a total physical examination.
Demonstrates substantial knowledge of interviewing and counseling techniques.
Diagnoses, treats, and plans appropriate health care for clients in school-based health care, women's health care, family planning, and primary care as appropriate for specialty; refers clients to physicians and / or outside agencies as required.
Demonstrates skillful knowledge of nursing techniques and procedures, medical and lab equipment and testing procedures, and medication.
Provides and prescribes medications within the area of specialty.
Performs laboratory procedures and interprets lab test results.
Provides case management services for select clients.
Ability to provide group, individual, and community education classes on birth control methods, sexually transmitted diseases, and other health concerns.
Establishes and maintains client medical charts and other necessary records.
Provides in-service education classes on assigned responsibilities for clinical staff.
Participates in Clinical Quality Improvement and Quality Management meetings and other meetings as requested by the supervisor and / or Senior Leadership Team.
Demonstrates ability to work independently, communicate effectively both verbally and in writing, deal tactfully and patiently with clients, and maintain effective working relationships with other related agencies, professionals, and citizen groups.
Promote individual and organizational integrity by exhibiting ethical behavior to maintain high standards.
Maintains professional affiliations and enhance professional development to remain abreast with professional literature and current healthcare management trends and rules in FQCH regulation.
Performs any other duties needed to help drive our Vision, fulfill our Mission, and abide by our Organization s Values.
Demonstrates skill in diagnostic procedures for diseases and the drug therapy needed for treatment.
Proficient in the use of medical and laboratory equipment.
Language Skills : Able to communicate effectively in English, both verbally and in writing; additional languages preferred.
Qualifications
Bachelor's degree from an accredited school of nursing : Master's degree in nursing; completion of a state-acceptable Nurse Practitioner's program; a minimum of one year s experience is desirable. National certification in a specialty is preferred.
Necessary special requirements : Licensed by the State of Oregon as a Registered Nurse, certified by the State of Oregon as a Nurse Practitioner, and Prescriptive privileges.
Ability to be credentialed with Medicaid, Medicare, private and other insurance.
Maintain confidentiality in accordance with WCHC policies and HIPAA regulations.
