Water’s Edge Health and Rehab is hiring a passionate Social Worker with healthcare experience to join our social services team in Kenosha, WI. As a Social Worker, you will work closely with our residents to ensure quality care is provided to improve the lives of our residents.
This position is Full-Time Monday through Friday with occasional evening hours as needed.
Duties and Responsibilities:
- Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Work with the finance team to ensure residents are educated and the proper documents are in place for the residents during their stay
- Work with the residents on discharge and get necessary DME or home care in place so that the residents are safe when they return to the community
Qualifications/Requirements:
- Degree in Social Worker
- Licensed Social Worker (LSW) preferred
- At least one year’s experience in Social Work in Long Term Care or similar settings
- Experience working with a diverse ageing population
