Job Title :

Physical Therapy Assistant

New graduates accepted; Skilled Nursing Facility experience highly preferred

License :

State License required

Certifications :

8 hours of Dementia Training

Must-Have :

Completed prescreen form
Attached resume
License verification

Job Description :

Ensure prescreen is filled out and attached upon submitting application.

Resume and license verification must be completed and attached; failure to do so will result in rejection of the application.

Experience in a Skilled Nursing Facility (SNF) is highly preferred for this role.

Conduct thorough State license verification.

Provide three professional references.

Undergo county criminal background checks for all counties lived and worked in the past seven years.

Complete federal and multi-state criminal searches for the past seven years or more.

Execute nationwide sex offender search covering the past seven years.

Perform Health and Human Services / Office of Inspector General (HHS / OIG) and General Services Administration (GSA) searches.

Ensure Social Security Number trace is completed.

Undergo 1-step PPD or Quantiferon test or provide proof of positive PPD and a chest X-ray.

Perform SAM and CORI Massachusetts background checks.

Position urgency is normal, and bilingual or holiday coverage is not required.
