Mental Health Case Manager
Job Summary
The Case Manager is responsible for providing case management services to clients in a private practice setting. The Case Manager works with clients to identify and resolve their needs, while promoting self-help and recovery. The Case Manager works collaboratively with other health care professionals, treatment providers, and support services to achieve the best outcomes for each client.
Essential Duties & Responsibilities
- Serves and protects the agency to professional standards, agency policies and procedures, federal, state local requirements, and CARF standards.
- Assists clients with accessing needed treatment and supports clients in achieving recovery from their assigned condition(s).
- Assists clients in developing positive attitudes toward goals and self-help activities.
- Receives referrals and comprehensively assess client needs and appropriateness with clinical knowledge
- Monitor and evaluate plan of care over time and complete and submit documentation on time.
- Develops individualized treatment plans for each client and assists in maintaining client’s progress toward desired goals.
- Maintains accurate records of client status, progress, and results.
- Participates in staff development activities within the private practice setting.
- Performs other duties as assigned.
Skills & Qualifications
- High school diploma or equivalent experience preferred.
- Experience working with individuals with mental health issues preferred.
- Ability to work independently and as part of a team environment.
- Strong interpersonal skills, including ability to communicate effectively with individuals and groups (e.g., families, friends, coworkers).

Job Type: Full-time
Pay: $20.00 - $35.00 per hour
