Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will be responsible for developing individualized service plans and ensuring clients receive the necessary support to achieve their goals.
Responsibilities
- Conduct comprehensive assessments to identify client needs and develop tailored service plans.
- Provide crisis intervention and management to clients facing immediate challenges.
- Facilitate access to community resources and services, ensuring clients are well-informed of their options.
- Utilize motivational interviewing techniques to empower clients in their decision-making processes.
- Collaborate with multidisciplinary teams to address complex client needs effectively.
- Maintain accurate case documentation and ensure compliance with all relevant policies and regulations.
- Support clients dealing with addiction issues through counseling and referral services.
- Advocate for child welfare by connecting families with appropriate resources and support systems.
Requirements
- Bachelor’s degree in Social Work or a related field; Master’s degree preferred.
- Proven experience in crisis management, social work, or related fields is highly desirable.
- Strong knowledge of community resources, including those related to childcare and workforce development.
- Excellent interpersonal skills with the ability to build rapport with clients from diverse backgrounds.
- Ability to work independently as well as part of a team in a fast-paced environment.
- Strong organizational skills with attention to detail in case documentation and reporting.

Job Type: Full-time
Pay: $40,000.00 - $50,000.00 per year
