- Provide social work services to students in accordance with student IEP's and accepted best practices.
- Conduct social assessments and functional behavior assessments, if needed.
- Complete all necessary paperwork according to federal, state and district guidelines.
- Monitor student progress towards IEP goals and collect data.
- Collaborate and consult with teachers, other related service providers and administration, as needed.
- Communicate with parents/guardians regarding student’s services and progress, as needed.
- Attend meetings and professional development, as directed by Diverse Learner Administration or Site Administration
- Perform other duties as assigned by the Diverse Learners Administration and/or Site Administrator.
- Provide nursing services to students in accordance with accepted practices and procedures as outlined in students’ IEPs.
- Perform health assessments, if needed.

Waukegan Community Unit School District No. 60 serves nearly 15,500 students in preschool through grade 12 and is located halfway between Chicago, Illinois and Milwaukee, Wisconsin on the shores of Lake Michigan. Our District has one early learning center, 15 elementary schools, five middle schools and a high school program divided between two 9 – 12th grade campuses. Responsibilities:
Qualifications: Professional Educator's License with a School Social Worker endorsemant.
Salary: Per contract
Additional Information: No more than two days can be missed without jeopardizing dismissal and termination of the contract unless extenuating circumstances prevail. An hourly rate of $35 per hour will be deducted for each hour or partial hour missed. This includes time before and after the start and end of the school day and all professional development sessions. In the event that a minimum class size is not maintained, the teacher is subject to release from their contract. The teacher will be paid for each day of service prior to the day of release.
