What are the responsibilities and job description for the LPN for Alzheimer's and Dementia Navigator position at Waveny LifeCare Network?
Reporting directly to the Executive Director of the Waveny Navigator Alzheimer's and Dementia At-Home Program (ADAH), the Navigator provides assistance and navigation services to clients enrolled in the ADAH program. This individual is a main point of contact for clients. The Navigator builds strong relationships with healthcare providers within the community that can assist, direct, and/or serve caregivers of clients suffering from Alzheimer's disease and other types of dementia. The Navigator is committed to removing the client's barriers to care by identifying critical resources for clients, helping them navigate through health care services and systems, promoting client health, serving as an advocate, educating clients on the disease progression, assisting the primary caregivers' with access to supportive services and coordinating care.
Responsibilities will include, but not limited to, quick and easy access to appropriate Physicians and Extenders, frequent communication and contact with clients, access to Respite Care and support services, and Clients' Rights.
Qualifications:
LPN or Social worker
- 2 years' experience in geriatrics or memory care,
- Understand Waveny's continuum of care
- Able to navigate complex healthcare issues and structures.
College degree preferred

· Kind, compassionate, trustworthy, and empathetic when working with clients and their primary caregivers,
· Technologically proficient,
· Ability to build relationships with different types of clients and their primary caregivers, healthcare and service providers,
· Good communication and interpersonal skills and ability to speak calmly, respectfully, and concisely with clients and Team members,
· Organized with confidential client material and appointment tracking,
· Flexible and adaptable in response to changing client and healthcare providers' needs
· Interest in working with Alzheimer's clientele and their primary caregivers.

Pay: From $70,000.00 per year
