What are the responsibilities and job description for the RN Home Care Case Manager position at Waveny LifeCare Network?
Waveny LifeCare Network is currently looking to hire Registered Nurses (RN) for our homecare division. In this role the individual will plan, organize, and direct home care services and is experienced in nursing, with emphasis on community health education/experience. They will be responsible for building from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.Responsibilities will include but are not limited to:
- Completes an initial assessment of client and family to determine home care needs.
- Provides a complete physical assessment and history of current and previous illness(es)
- Regularly re-evaluates client care needs during all visits and specific prescribed times.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing actions. Includes the client and family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the State of CT Nurse Practice Act.
- Counsels the client and family in meeting plan of care outcome objectives.
- Provides health care instructions to the client as appropriate per assessment and plan of care.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharging of the client.
- Assumes role of Case Manager for all assigned cases and assumes responsibility to coordinate client care for assigned caseload.
- Attends all staff meetings or reviews agenda to keep abreast of changes/updates.
- Participates in staff meetings to share skill set, ideas, and suggestions.
- Documents according with prescribed agency tools in timely fashion.
- Prepares clinical notes and updates the primary physician when necessary and according to policy, and at least every 60 days.
- Communicates with the physician regarding client needs and reports any changes in client’s condition, obtains/receives physician’s orders as required
Qualification for this role:
- 1. Graduate of an accredited school of nursing. Bachelor’s degree preferred. One (1) to two (2) years of recent acute care experience is an institutional setting is preferred or recent home care experience.
- Current RN licensure in state, CPR if applicable and valid driver’s license.
- Able to provide supervision to home health aides.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist.
- Computer knowledge helpful but not required.
- Prolonged or considerable walking, standing. Able to lift position or transfer clients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills. Driving in all weather conditions.
