About the Role
We are currently seeking a Physical Therapist to join our team. As a Physical Therapist at Waverly Rehabilitation And Healthcare Center, you will be responsible for providing comprehensive physical therapy evaluations and treatments to patients, as ordered by their physicians.
Responsibilities include:
- Conducting thorough physical therapy evaluations based on physician orders
- Documenting evaluation findings accurately and timely
- Developing and implementing individualized treatment plans, including short- and long-term goals, frequency, duration, and modalities
- Ensuring physician orders are obtained for evaluations, treatments, re-certifications, and discharges
- Providing evidence-based physical therapy interventions to patients, modifying treatment plans as needed while adhering to precautions
- Documenting patient participation and progress on weekly progress notes accurately and timely
- Updating treatment plans as necessary
- Documenting patient discharge summaries and in-service forms as required
Requirements
To be successful in this role, you must have:
- A graduate degree from an accredited university with a Bachelor of Science, Doctorate, Master of Arts, or Master of Science in Physical Therapy recognized by the American Physical Therapy Association (APTA)
- A current license and/or registration in the state where the facility is located (as applicable)
