Compensation:
- In accordance with California law, the expected salary range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.
Role Summary
As a Case Manager, you will be pivotal in supporting children, youth, and families facing life’s toughest challenges. Whether you provide hands-on case management, help to develop life skills, or provide emotional support, your work will directly impact the lives of those we serve.

How you will Impact Change:
- Acts as liaison between residents, families, service providers, programs and the community for all DCFS case management domains.
- Create and maintain referral tracking system that identifies and tracks the referral and intake process through its various phases up to and including the 30-day follow up Needs and Service Plan (NSP) with the appropriate treatment team.
- Create and maintain client data tracking system that identifies: demographical information, program stay duration, services received and other significant datapoints to assist with TSCF quarterly reporting requirements as specified by oversight entities.
- Complete iTrack documentation in accordance to Los Angeles County DCFS Incident Tracking System for all residents for the following: school refusals (not enrolled within 72 hours of compliance), reports of child abuse by resident as required by mandated reporting and other significant events required to be inputted into the system.
- Schedule and facilitate from date of intake and on-going residents 30-day Child Family Team Meeting (CFTM) to ensure continue care coordination is maintained on the behalf of the resident while in temporary shelter care to permanent placement transition.
Qualifications
Education and Credentials
- A Bachelor’s degree in a related field from an accredited college or university is required.
- Minimum two years of experience working as a Case Manager, preferred.
- A minimum of 1-2 years of experience working with at risk severely emotionally disturbed (SED) youth and/or non-minor dependents required.
- Knowledge of local regulations, state laws, and procedures
- Knowledge of social casework principles and methods.
- Considerable knowledge of the Regional Center and Department of Children and Family Services systems, and Supplemental Security Income (SSI) and Medi-Cal resources.
- Minimum 1 year of experience making effective use of community resources, establishing and maintaining an effective working relationship with children, families, staff and outside agencies.
