PRIMARY RESPONSIBILITIES

1. Under the general guidance and supervision of the SSA Director and SSA Supervisor, the Assessment Specialist Service Coordinator is responsible for the coordination and administration of formal assessments withing SSA department. This includes, but not limited to, conducting Waiting List Assessments (WLA), Extraordinary Care Assessments, complete COEDI and OEDI assessments for county board eligibility as requested by the Intake Department, and Pre-Admission Screening Resident Review (PASRR) assessments. Additionally, the Assessment Specialist Service Coordinator will participate in state or local hearing process for any appeals for WLA’s. This role may require maintaining a minimal caseload, depending on the needs of the SSA department. In addition, the Specialist will serve as a backup support to the SSA department, assisting with caseloads as necessary, and working under the direction of the SSA Supervisor or SSA Director.

ESSENTIAL FUNCTIONS

Waiting List Assessments: Complete the DODD wait list assessment per DODD timelines through a face-to-face assessment to determine if identified current needs are being met by alternative resources or if the identified person needs to be placed on the wait list. The WLA will be administered when an individual, guardian, or member of the family: 1. Requests the county board administer the waiting list assessment; 2. Requests the individual be enrolled in a HCBS waiver or placed on the waiting list; or 3. Identifies an unmet need. A WLA will be readministered when the individual, guardian, or family member identifies change in circumstances that may result in having a current or immediate need. Once the DODD wait list assessment is completed, the specialist collaborates with the SSA to review the results and submit to the SSA Director for finalization of the assessment. The Assessment Specialist Service Coordinator will notify the individual/guardian/family in writing of the outcome of the assessment, a copy of the complete WLA tool and explanation and give due process. Assessment Specialist Service Coordinator will participate in state or local hearing process for any appeals for WLA’s.

Extraordinary Care Assessments: When a parent of a minor child or spouse requests to become a paid caregiver on or after 1/1/24, they will be directed to the Assessment Specialist Service Coordinator. The assessment specialist will contact the parent to discuss the needs of the individual and complete the Extraordinary Care Instrument through a face-to-face assessment. The instrument must be completed on a recurring basis annually or whenever there is a significant change in the individual’s needs or condition.

Intake and Eligibility – As requested by the Intake Department and under the direction of the SSA Supervisor or SSA Director, complete COEDI and OEDI assessments for county board eligibility. Assess and evaluate the eligibility of individuals requesting services under the COEDI and OEDI process. Maintain ongoing contact, review case notes, generate reports, initiate email correspondence, written correspondence, perform records reviews and documentation of follow-up on unusual

incidents, and other necessary records/reports in a timely and accurate manner consistent with agency and regulatory standards.

Preadmission Screening Resident Review (PASRR): As requested by the Intake Department and under the direction of the SSA Supervisor or SSA Director, complete the PASRR once the county receives notice of a possible admission or an admission into Medicaid certified nursing facility. Coordinate and collaborate with DODD, nursing home, Service and Support Administrator, hospital, and any other entity that has the required documentation that is needed for the completion of the PASRR.

MISC: Other duties as assigned by the SSA Director and SSA Supervisor or to meet the needs of people supported by SSA Department or the overall WCBDD. A small caseload could be assigned to support the SSA department during the leave of absence by an SSA or any other changes in the department that might result in a person receiving support not having an assigned SSA. Attend required training and seminars; maintain SSA certification.

Back up SSA Department Support/Caseloads: When working with a minimal caseload, the Assessment Specialist SSA will perform job duties of a SSA including, but not limited to:

  • Develop Individual Service Plan and facilitate interdisciplinary team process in the approval of the ISP; Ensure maximum use of community and natural resources in addressing the support and services identified as a want or need through informal and formal assessment; address and amend plans as necessary, through the team process, to address identified plans of action; provide training on the ISP for selected providers of the identified service and supports.
  • Assure assessments are completed in a timely manner and per DODD rule. Make appropriate referrals to outside agencies; work in collaboration with other professionals to identify needed services and supports; When working with a case, serve as the main contact for any crisis assistance; work with individual/guardian to ensure choice in provider selection; Ensure appropriate paperwork is completed to enroll/continue enrollment on a HCBS waiver
  • Ensure the implementation of the approved ISP, through monitoring of the services and supports identified through a review of the assigned caseload, as identified per team process, to determine individual level of satisfaction and progress on any identified outcomes; meet with individuals on the assigned caseload at intervals determined by the supervisor; report findings of monitoring to supervisor and make any necessary revisions to the ISP.
  • Serve as the main contact for any crisis assistance needed by the individuals on their identified caseload. Ensure all reasonable measures to protect the health and safety of individuals involved are taken.
  • Document activities in case notes, meeting the standard of productivity determined by the supervisor; maintain official file for individuals on assigned caseload

MINIMUM QUALIFICATIONS AND EDUCATION REQUIREMENTS

Eligible for Service and Support Administration Certification as identified in OAC 51232-5-02.

PREFERRED SKILLS AND COMPETENCIES

Knowledge of counseling and interviewing techniques, human relations, public relations, and office practices and procedures. Knowledge of Ohio rules and laws.

  • Use Facilitation of Group Process techniques to facilitate team process
  • Develop well-informed advice and strategies based on analysis of issues and trends.

  • Engage and mobilize others in developing goals, executing plans, and delivering results by communicating clearly and consistently; Use negotiation skills and flexibility to encourage recognition of joint issues, collaborating across boundaries to influence the success of outcomes.
  • Ensure individuals/guardians have the tools and support needed to meet outcomes.
  • Collaborate professionally with internal staff and external partners.

Job Type: Full-time

Pay: $42,000.00 - $56,444.00 per year
