RESPONSIBILITIES: The Intake Specialist position serves as an initial point of contact for people seeking information regarding services provided by the Wayne County Board of Developmental Disabilities and assesses the need and qualification. The position requires one to complete the Children’s/Ohio Eligibility Determination Instrument (C/OEDI) for each individual seeking services, completing pre-admission screenings and resident reviews (PASRR) for individuals with developmental disabilities seeking admission to a nursing facility, and transfers of individuals in and out of Wayne County. The Intake Specialist will receive referrals, conduct evaluations, make recommendations regarding eligibility and provide service coordination. The job will also require the ability to coordinate and conduct intake meetings utilizing principles of person centered planning and transferring of services to an assigned Service and Support Administrator.
ESSENTIAL FUNCTIONS
- Receives referrals through intake process. Provides information and referral services to adults, children and their families. Arranges evaluations, interviews individual and appropriate supports, analyzes individual’s needs and/or refers to appropriate agencies/services. Assures intake procedures and the Board eligibility criteria are clear and understood.
- Collects necessary information to facilitate intake and eligibility determination and determines eligibility for services using Ohio Department of Development Disabilities and Board approved procedures.
- Coordinates and participates in the intake meeting and includes all necessary members while utilizing person centered planning.
- Completes all necessary paperwork in an accurate and timely manner. Documents services in accordance with established Targeted Case Management (TCM) guidelines.
- Coordinates PASRR reviews as directed.
- Maintains information/referral files/systems on an as needed basis including intake tracking log. Develops procedures and systems for intake, eligibility and referral process as well as maintains necessary forms.
- Provides outreach in eligibility of Board Services to Community agencies and businesses as requested.
- Maintains contact with as well as updated current knowledge of the mental health system, school districts, Opportunities for Ohioans with Disabilities, and other services and agencies to ensure information is available.
- For children 3 to 5 years old, obtains the evaluation completed by or for the school district or medical and/or psychological evaluations to determine the presence of a developmental disability.
- For individuals ages 6 and up, obtains collateral information to establish the presence of a developmental disability, including school, medical and/or psychological evaluations. Arranges interviews with all necessary sources.
- Participates in in-services, course work, conferences, seminars and other educational programs to increase knowledge, information and skills and to maintain certification while promoting professional growth.
- Participates in community outreach by interacting with and educating a variety of agencies, individuals and groups about eligibility requirements and process.
- Performs all other related work as assignments as assigned by the Superintendent or the Director of Service and Support Administration.
MINIMUM QUALIFICATIONS AND EDUCATION REQUIREMENTS
Associate’s or Bachelor’s Degree
KNOWLEDGE, SKILLS AND ATTRIBUTES (Attained through coursework or experience)
· Knowledge of interdisciplinary team methods, departmental policies, and procedures.
· Knowledge of ISP content, construction, procedures, and implementation.
· Knowledge of basic data entry including but not limited to Microsoft Office (Word, Power Point, and Excel, and Outlook) and Portable Document Format.
· Ability to maintain a high level of accuracy and be timely with data and documentation.
· Efficient with basic office equipment, including but not limited to phones, fax, and computers.
· Demonstrate ability to employ effective organizational skills, and effective verbal, listening, and written communication skills
· Demonstrate analytical and problem-solving skills, interpersonal skills, stress management and time management skills
· Maintain strict confidentiality concerning sensitive material and be compliant with HIPAA compliance; possess cultural awareness and sensitivity
· Be prompt and organized.

Pay: $42,000.00 - $56,444.00 per year
