Job Description:
Position: Social Service Designee Department: Admin
Hours: Day shift (40 hours)
Days: Full time 8a-4:30p (occasional evening and weekend appts. or events)
The Wayne County Care Center is currently accepting applications for the position of full time Social Service Director for the County’s 50 bed skilled/ intermediate nursing facility.
Responsibilities of the Social Service Designee:
- Record and maintain regular progress notes indicating response to treatment plan and/or adjustment to life in the facility.
- Provide consultation to solve the needs and problems of the residents through the development of social service programs.
- Assist in obtaining resources to meet the needs of the residents.
- Evaluate and assist in determining a plan for social treatment.
- Maintain contact with the resident’s family, involving them with non-medical reports.
- Make routine visits to the residents and provide services as necessary.
- Coordinate social service activities with other departments.
- Evaluate potential admissions and assist with tours and admission paperwork
- Communicate with other staff to ensure residents are provided with the best experience possible
- Ensure that staff are mentored to enforce resident rights.
- Must participate in facility inspections as needed and perform administrative duties.
Requirements of Social Service Designee:
- Licensed social worker preferred
- Minimum 1-year health care experience
- Experience in Point Click Care preferred
- Compassion for and commitment to hospitality, service and excellence in senior care
- Ability to be a responsible and dependable team member who can take direction well
- Adaptable and able to work under pressure
