DESCRIPTION OF WORK
As an Aging Care Manager 3, your responsibilities will include maintaining a specialized caseload of vulnerable consumers who have been determined to be in an abusive, neglectful, abandoned, or exploited situation. You may also carry a caseload of consumers who are not classified as needing Protective Services or who are not classified under the law as protective service consumers but are still in need of assistance from the agency. Our consumers will rely on you to make critical decisions and encourage those who would greatly benefit from positive change in their lives.
Your duties will involve coordinating medical, social, psychological, and functional evaluations for consumers assigned to your caseload as well as conducting assessments and investigations within required timeframes. Additionally, you will be expected to appear before the County Judge for intervention hearings to testify or for a behavioral health commitment and can arrange, if needed, a representative payee to protect the consumer's financial assets and resources.
Success in this role relies on your initiative, problem solving skills, close attention to details, and ability to work and respond well in an emergency setting. Extensive knowledge of the community resources available is also key for effectively assisting consumers. All duties for this position are performed in accordance with strict procedures, reporting deadlines, and work performance standards to help ensure the safety and well-being of our consumers. Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch. Employee may have irregular hours, including evenings, weekends, or holidays. Employee required to be on the on-call rotation.
- Employee may have overnight travel or out of County travel for meetings and trainings.
- Telework: You will not have the option to telework in this position.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
Other Requirements:
- You must be able to perform essential job functions.
Legal Requirements:
- You must pass a background investigation.
- A conditional offer of employment will require drug screening.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
