DESCRIPTION OF WORK
As a County Caseworker 1 in this position, you will develop plans for Intellectual Disability consumers following intake process. Develops treatment plans and enrollment into Behavioral Health/Intellectual Disabilities/Early Intervention/Autism programs as appropriate. You will also develop Individual Support Plans (ISP) with detailed goals and will locate providers as detailed on the ISP. The County Caseworker 1 maintains files, records and other documentation for the continuity of care in a timely and organized manner. Develops life management plans and acts as an advocate for the consumer to ensure appropriate treatment are received as approved. Documents contacts with consumer, family, service providers.

Work Schedule and Additional Information:
- Full-time employment; 37.5 hours/week.
- Work hours are 8:00 am to 4:30 pm, Monday - Friday, with a 1-hour lunch.
- May work more than eight hours per day.
- Employee may have occasional overnight travel or out of County travel for meetings or training.
- Frequent driving both in and out of County.
- ID Supports Coordinators are not on call.
- In an emergency may be called out.
- Telework: You will not have the option to telework in this position.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:
- Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
- A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
- Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
- Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.
Other Requirements:
- You must be able to perform essential job functions.
Legal Requirements:
- You must pass a background investigation.
- A conditional offer of employment will require a drug screening.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
