General Information
The Transitional Case Manager will provide transition planning services to clients in a residential setting who are nearing the end of the program. They will be responsible for documenting all actions according to State and Federal requirements.
Duties/Responsibilities:
· Focus on independent living skills and workforce training.
· Work directly with youth and other members of the treatment team to develop an individualized transition plan based on assessments and historical information.
· Complete appropriate reports, assessments, and reviews assigned in a thorough and timely matter as well as the related documentation.
· Perform related tasks in cooperation with the Program Director and in accordance with the requirements of the Provider Policy Manual and the Licensure Unit.
· Attend training and seminars as needed and approved to obtain or maintain licensure or to increase skills.
· Follow all applicable policies of Wayne Halfway House, Inc.
· Participate in the PQI program as requested.
· Demonstrate competence in cultural knowledge, awareness, and sensitivity.
· Perform any other job-related task as assigned by the Program Director.

Education and Experience
· Bachelor’s Degree from an accredited institution with a major in social work, psychology, counseling, special education, or a related human services field is required.
· Master’s Degree in the social sciences or volunteer, practicum or intern experiences in programs/facilities that work with children and families preferred but not required.
· At least five years experience in a juvenile facility is preferred but not required.
NO FELONY CONVICTIONS. Must pass a drug screen and background check and maintain eligibility for continued employment.
Job Type: Full-time
Pay: $50,000.00 per year
