Overview of the role
The Counselor or Therapist of the Wayspring Clinic will be responsible for providing therapy as a service to members identified by Wayspring Clinic providers. The Counselor of Therapist will collaborate with the multidisciplinary care team including medical and behavioral health providers on the plan of care for each member. The Counselor or Therapist will also coordinate with the SUD Home team for ongoing support from Care Partners, Clinical Care Navigators and to ensure each member has adequate access to services to best support them in their journey to wellness.
This is a hybrid role, working from home and within the clinic each week.
Responsibilities
- Creates individualized treatment plans according to patient needs and circumstances
- Meets with patient regularly to talk about treatment plan, track progress on treatment plan, and adjust treatment plan as necessary
- Conducts ongoing screenings of patient progress
- Complies with documentation standards within clinic documentation platform
- Collaborates with other members of the multidisciplinary care team within the Wayspring clinic, as well as other providers and facilities outside of the clinic as necessary
- Adhere to Wayspring information security and privacy requirements
Requirements & Preferred Qualifications
- CADC
- Must exhibit empathy, listening skills, social and communication skills, boundary setting, critical thinking, organizational skills, written documentation skills, technical/computer skills
- Experience working with Medicaid populations is highly preferred
Job Type: Full-time
Pay: $60,000.00 - $75,000.00 per year
