Our flexible schedule and flexible work environment options help you to create and maintain the work-life balance you need most.Overview of the Community Outreach SpecialistThe Community Outreach Specialist (COS) will report to the Member Engagement Manager and will be tasked with contacting eligible health members in the community to educate them on Wayspring's service offerings. The COS will utilize a prioritized list for outreach and have confidence navigating the complexities of the community. The ideal candidate will be a highly personable professional with excellent communication skills. The candidate will also have experience working with vulnerable populations who may be disengaged from healthcare and live a more transient lifestyle. This individual will work to support a specific market that Wayspring offers services in.Responsibilities of the Community Outreach SpecialistFacilitates and secures enrollments from members in the community to participate in Wayspring's servicesActs quickly to assess member needs and triage accordinglySuccessfully engages members and provides them with an educational overview of Wayspring's service offeringsSecures verbal and written consent, and obtains appropriate paperwork for services, including consent to clinic services and release of information forms, from members to participate in Wayspring's servicesSecures members' first appointment with clinical servicesProvides frequent touchpoints to enrolled caseload and ability to assess individual member needs to determine the frequency of touchpoints needed ongoingInitiates creative strategies to facilitate member contactIncorporates recovery support and hard reduction themes into engagementRegularly reviews member profile for SDoH and clinical updatesMaintains performance accountability around member enrollment and clinic conversion, and achieves performance targetsAdheres to Wayspring information security and privacy requirementsRequirements & Preferred QualificationsMinimum of three (3) years of experience in a community outreach role, with preferred experience around street outreachBachelor's degree or work experience equivalent in a relevant fieldDemonstrated ability working in a matrixed environmentThe ability to communicate effectively and persuasively is requiredExceptional verbal communication skillsExceptional verbal de-escalation skills and crisis managementExperience with CRM platforms is preferredAbility to travel as business needs require (community-based role).
