FAMILY SUPPORT SERVICE NAVIGATOR/ Case Manager

The Family Support Service Navigator builds strong relationships with individuals with disabilities and their families to support community participation and inclusion. Service Navigators support individuals and families with a variety of services based on the needs of each individual and family, including: information and referrals, service navigation, support groups, trainings, parent networking and mentoring, administration of flexible funding allocations, and the facilitation of social/recreational events.

SCHEDULE FOR THIS POSITION: 40 hours/week, schedule will vary based upon needs of the families but must include availability for one evening per week and occasional Saturdays

Responsibilities:

  • Provide information and referral services to families who contact the Family Support Center.
  • Connect and coordinate families with community programs of interest and services as needed.
  • Provide service navigation in areas of expressed need or interest of the family.
  • Partner with families to develop a comprehensive Family Support Plan and to support with identification of social, educational, and recreational activities, referrals, and resources as applicable to support the individuals inclusion and belonging in their community.
  • Partner with families in the planning of use of flexible funding allocations in accordance with DDS guidelines and Waystone policies including preparation and approval of appropriate documents.
  • Develop, staff, participate, and/or lead recreational, training, and support group opportunities provided by the Family Support Center as needed.
  • Communicate and coordinate with DDS Service Coordinators as needed, including providing verbal and/or written updates as requested.
  • Maintain and update individual database records on a regular basis.
  • Travel within the Merrimack Valley region as required for community events and meetings.
  • Attend and participate in regular meetings and ongoing communication and collaboration with Family Support Center and Children and Family Services teams, including providing verbal and written updates and reports to internal and external stakeholders as requested.

Requirements:

  • Bachelors degree with minimum of 2 years experience or an Associates degree with a minimum of 4 years experience in Human Services or related field
  • Strong written and verbal communication skills with focus on positive outcomes
  • Extensive knowledge of community services, programs, and resources for children, adolescents, and adults with intellectual and developmental disabilities and/or Autism Spectrum Disorder and their families.
  • Strong organizational skills and ability to multi-task.
  • Competent computer skills, including experience in Outlook, Word, Excel, and virtual platforms.
  • Experience working with families from a variety of cultural backgrounds and families experiencing social and economic challenges, such as substance use disorder or housing instability.
  • Valid drivers license with minimum of 1 year driving experience.
  • Regular and predictable attendance.
  • Bilingual in Spanish and English, preferred.
