WCS Healthcare has an immediate LPN opening with a large Social Services agency working with Adults and Families in Shelters!

Job Function

• Take vital signs, such as blood pressure, temperature and weight
• Assist with diabetes medication and treatment
• Take medical histories and enter information in computer systems
• Coordinate with Case Managers, Senior Case Managers and Director of Social Services to ensure proper care, nutrition and treatment plans are followed
• Follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed
  
  

Job Requirements

• Completion of accredited program and licensed in New York as an LPN
• Preference is given to those who have experience working on a multi- disciplinary team and/or previous experience with homeless population
• Ability to work independently and as part of a team, to assist individuals of diverse ethnicities, backgrounds, ages, and sexual preferences
