The Direct Care Worker provides compassionate, hands-on support to clients in their homes, assisting with personal care needs, daily living activities, and promoting a safe, healthy environment. This role emphasizes fostering client independence, dignity, and well-being.

Key Responsibilities
- Follow the plan of care to ensure safe, competent, and effective support for clients.
- Assist clients with personal hygiene tasks, including bathing, grooming, and toileting.
- Support clients with ambulation and mobility as instructed by the care plan.
- Plan and prepare nutritious meals and shop for groceries, as directed.
- Perform light housekeeping tasks to maintain a clean and comfortable living environment.
- Encourage clients to participate in activities that promote mental alertness and independence.
- Provide emotional support and build trusting relationships with clients and their families.
- Monitor and report changes in clients' mental or physical condition to the supervisor.
- Accurately complete and submit daily visit documentation in a timely manner.
- Attend in-service training sessions to maintain compliance with agency requirements.
Qualifications
- High school diploma or equivalent preferred.
- Successful completion of one of the following training programs:
- A minimum of 75 hours of training and competency evaluation as specified in 42 CFR 484.80.
- An agency-developed training program meeting PA Code 611.55 criteria.
- A competency evaluation program that meets 42 CFR 484.80 requirements.
- A nurse aide training and competency evaluation program approved by the state.
- Knowledge of personal care, household management, and meal preparation.
- Understanding and respect for client rights, ethics, and confidentiality.
- Current CPR certification (online certifications not accepted).
- Successful criminal background check.
- Reliable transportation is preferred; bilingual proficiency is a plus.
