The Role
We are seeking a compassionate and skilled Speech Therapist to provide services to home care clients experiencing speech, language, or swallowing difficulties. Working under the direction of the attending physician and in collaboration with the interdisciplinary care team, you will help clients achieve their functional goals and improve their overall quality of life.
What You’ll Be Doing
- Performing comprehensive speech therapy assessments, diagnostic tests , and skilled treatments under a medically approved Plan of Care.
- Developing and implementing individualized care plans to address functional speech deficits and swallowing difficulties .
- Collaborating with physicians and interdisciplinary teams to ensure cohesive, high-quality care.
- Educating clients and their families to support treatment goals and evaluate the effectiveness of therapy plans .
- Supervising home health aides providing services related to speech therapy plans.
- Documenting client progress , plans , and outcomes in compliance with Agency policies and regulatory standards.
- Promoting safety and maintaining confidentiality in all aspects of client care.
