Summary
The Case Manager is responsible for managing all aspects of the patient’s care, treatment, and services throughout the continuum of care.
Educational Background and Experience:
- 2 years of experience working as a case manager in substance abuse center, psychiatric facility, or health care-related and/or recovery setting.
- Bachelor's degree in social work, social services, psychology, nursing, or other mental health related field. Individuals with a master’s degree will be given priority. In lieu of a degree, five or more years of relevant experience will be considered.
- Credentialed or Professional License relevant to degree or field of study will be given priority; credentialing will be encouraged.
- CPR - Must have prior to the time of hire.
Job Requirements
- Case manager will ensure that the patient is safely transitioned from admission, treatment, discharge planning and aftercare in a manner that ensures access to the best and most effective treatment and care alternatives.
- Services include, but are not limited to, biopsychosocial assessment, interdisciplinary treatment planning, participating in interdisciplinary treatment team, coordinating family outreach and family Zoom sessions, social services support, discharge planning, and aftercare follow up.
- Interventions may include return to work or school coordination, applications for programs or services, assistance with completing FMLA or disability forms, coordination of follow up services, and interaction with governmental agencies when required.
- Position requires concurrent chart audits for timely and effective documentation as well as coordination with utilization review departments for insurance reviews.
- Other duties as assigned.

Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
