As an MDS Coordinator at Webster County Nursing Homes, you will play a vital role in the assessment and care planning process for our residents. This position is essential in ensuring compliance with regulatory standards while enhancing the quality of life for our residents.
Responsibilities
- Conduct comprehensive assessments of residents using the Minimum Data Set (MDS) to determine their needs and develop individualized care plans.
- Ensure compliance with federal and state regulations regarding MDS assessments and documentation.
- Collaborate with interdisciplinary teams to coordinate resident care and services effectively.
- Monitor resident progress and adjust care plans as necessary based on ongoing assessments.
- Provide training and support to staff on MDS processes and documentation requirements.
- Maintain accurate records of assessments, care plans, and related documentation in accordance with facility policies.
- Participate in quality improvement initiatives to enhance resident outcomes and facility performance.
Qualifications
- Current nursing license (LPN or RN) in the state of Missouri or equivalent qualifications.
- Proven experience in case management or utilization management within a healthcare setting.
- Strong knowledge of MDS assessment processes and ICD-10 coding guidelines.
- Excellent communication skills to effectively interact with residents, families, and team members.
- Detail-oriented with strong organizational skills to manage multiple tasks efficiently.
- Experience in long-term care or skilled nursing facilities is preferred but not required.
- Ability to work collaboratively within a multidisciplinary team.
