Job Summary
The Wound Care Nurse plays a critical role in the assessment, treatment, and management of patients with complex wounds. Duties
- Conduct thorough assessments of patients' wounds and overall health status.
- Develop individualized care plans based on assessment findings and patient needs.
- Implement wound care treatments, including dressing changes, debridement, and infection control measures.
- Educate patients and their families on wound care techniques, prevention strategies, and self-management.
- Collaborate with healthcare team members to ensure comprehensive patient care.
- Maintain accurate documentation of patient assessments, interventions, and outcomes using electronic health record systems such as Epic.
- Monitor patients for signs of complications or changes in condition that may require adjustments to the care plan.
- Provide health coaching to promote overall wellness and recovery.
- Adhere to established standards of care and protocols for wound management.
Skills
- Proficient in performing physical examinations and assessing wound characteristics.
- Knowledgeable in critical care nursing principles and practices.
- Experience with hospice palliative medicine for end-of-life wound management.
- Familiarity with tube feeding procedures when applicable to patient care.
- Ability to perform spinal taps as needed for diagnostic purposes.
- Strong communication skills to effectively educate patients and collaborate with healthcare teams.
- Detail-oriented with a focus on maintaining high standards of patient safety and quality care.
- Ability to work effectively within established systems while adapting to the needs of individual patients.
- Experience in infant care is a plus but not required.
